Characteristics of Kitano Hospital's home rehabilitation services

What is done in home rehabilitation?

• Disease management (checking the health of people with heart or respiratory diseases, etc.)
• Adjusting the amount of exercise according to the condition
• Proposals for improving activities of daily living
• Adjusting dietary content according to swallowing ability
• Individual independent exercise instruction

Eligible patients

• Those who have been certified as requiring nursing care after being discharged from our hospital
  *Separate consultation is required for home rehabilitation covered by medical insurance.
• Patients who live within approximately 4km of Kitano Hospital
  Kita Ward, Miyakojima Ward, Yodogawa Ward (part), Chuo Ward (part)
• The illnesses include stroke, orthopedic surgery, chronic heart failure, and chronic respiratory failure.

Points to note

• This is limited to a period of three months after discharge from hospital.
• If rehabilitation needs to continue after three months, we will discuss how to continue it in advance with you.
• Visits are scheduled for 40 minutes per day, 2-3 times per week.
• During your hospitalization, we will make preparations for both home rehabilitation and transfer to a rehabilitation hospital.
