Perianal abscess (infantile anal fistula)

This is a disease in which the area around a baby's anus becomes red and swollen, and pus accumulates. It often occurs between about one month and one year of age. The pus that has accumulated under the skin often flows out through the skin, and once the pus is out, the swelling will temporarily improve. However, pus may repeatedly accumulate, causing the area to become red and swollen. The area around the anus may also become red and swollen in multiple places.

What is the cause?

Deep inside the anus there are many ducts that secrete mucus that helps the feces pass smoothly. Bacteria from feces can enter these ducts, multiply deep inside, and cause pus to accumulate around the anus. This condition in which pus leaks from around the anus is called anal fistula. As anal fistula is seen in infants, it is commonly called infantile anal fistula. It is said to be related to the incomplete immune system during infancy, but the reason is not fully understood.

What is the treatment?

If the swelling is large, we will make a small incision in the outpatient clinic to drain the pus. The pus will accumulate and be drained repeatedly, but most will heal naturally by around the age of one. The release of pus is good for the body, so be patient and wait for it to heal.

There is no need to take antibiotics or disinfect the area around the anus. Just wash it thoroughly and keep it clean. There are also reports that Chinese herbal medicines are effective.

What about anal fistulas after infancy?

Most anal fistulas in infants will heal naturally, but some may not heal even after the child turns one year old. In such cases, surgery may be required. As with adults, anal fistulas in older children may require surgery.
