Our hospital has opened an admission and discharge support station to ensure that patients and their families can have a safe and secure hospital stay. Here, patients will meet with staff including nurses, pharmacists, and registered dietitians before being admitted. Through collaboration among multiple professions, we provide medical care that respects the thoughts and wishes of patients and their families, and support patients so that they can continue to receive appropriate medical care from relevant social and community institutions.
The role of the admission and discharge support station
Patients scheduled for admission will be interviewed by the following professionals:
nurse

- Confirm the patient's basic information (emergency contact information, daily life, nursing care insurance, etc.).
- Explanations about preparations for hospitalization and recovery life.
- Information about the schedule during hospitalization.
- Checking concerns about hospitalization and life after discharge.
- Collaborate with nurses, social workers, outpatient departments, and ward departments at the local medical service center.
pharmacist

- Check the medications you are currently taking (including eye drops, patches, inhaled medications, supplements, and health foods).
- Explains which medications to discontinue if instructed by a doctor in order to safely carry out surgery or examinations.
- Explains supplements, health foods, and over-the-counter drug discontinuation to provide safe treatment.
Registered dietitian

- Food allergy confirmation.
- Check height and weight.
- Check your regular eating habits.
For outpatients who have been admitted to hospital
- After your outpatient consultation, you will be required to undergo a pre-admission examination.
- If the examination results indicate that you require inpatient treatment, you will be asked to meet with the Admission/Discharge Support Station for an interview and to apply for hospitalization.
For those undergoing surgery
- If you have decided to have surgery, we will check in advance to make sure that any medications you are taking will not affect the surgery.
*We will contact you in advance if you need to stop taking any medications.
- You will be examined by an anesthesiologist beforehand.
- The admission and discharge support station will explain the necessary items and precautions.
For those undergoing testing while hospitalized
- If you are hospitalized and undergoing testing, the precautions to take regarding the test will be explained in advance at the Admission and Discharge Support Station.
- If you need to check your medication, a pharmacist will check it for you.
