The Kitano Hospital, a public interest incorporated foundation run by the Tazuke Kofukai Medical Research Institute (hereinafter referred to as "our hospital"), has established the following privacy policy (hereinafter referred to as "this policy") regarding the handling of privacy information in the "No Waiting, Easy" service (hereinafter referred to as "this service") it provides.

Article 1 (Privacy Information)

"Personal information" in the privacy information section refers to "personal information" as defined in the Personal Information Protection Act, which is information about a living individual that can identify a specific individual based on the name, date of birth, address, telephone number, contact details, credit card information, and other descriptions contained in the information.

Article 2 (Method of collecting privacy information)

1. When users register, our clinic will not ask for personal information such as name, date of birth, address, telephone number, email address, or credit card information.
2. We may collect payment records, including personal information of users, made between users and our business partners, etc. from our business partners.

Article 3 (Purpose of collecting and using personal information)

1. The purposes for which we collect and use personal information are as follows:
   1. To display information such as name, address, contact details, credit card information, services used and their medical fees, so that users can view and correct their registered information and view their usage status.
   2. To use information such as name, date of birth, address, and telephone number to verify the identity of the user
   3. To use payment information such as the duration of treatment, number of treatments, amount billed, name, address, etc., in order to bill users for medical expenses.
   4. To display the information registered with our clinic on the input screen so that users can easily input data, and to transfer it to other services (including those provided by our partners) based on the user's instructions.
   5. The purpose of using information that can identify individuals, such as usage status, name, and address, in order to deny use to users who have violated the terms of use of this service, such as by delaying payment of medical fees or causing damage to a third party, or who attempt to use the service for fraudulent or improper purposes.
   6. To respond to inquiries from users, we will use information necessary for our hospital to provide services to users, such as the content of the inquiry and information related to medical fees, etc., as well as the user's service usage status and contact information.
   7. Purposes incidental to the above purposes of use

Article 4 (Provision of personal information to third parties)

1. Our clinic will not provide personal information to third parties without the user's prior consent, except in the following cases, unless permitted by the Personal Information Protection Act or other laws and regulations.
   1. When required by law
   2. When it is necessary to protect a person's life, body, or property, and it is difficult to obtain the individual's consent.
   3. When it is particularly necessary for the improvement of public health or the promotion of healthy child development, and it is difficult to obtain the consent of the individual.
   4. When it is necessary to cooperate with a national government agency, local government, or a person commissioned by them to carry out duties prescribed by law, and obtaining the individual's consent may impede the performance of said duties.
   5. When the following matters have been announced or publicly announced in advance:
      1. The purpose of use includes providing the information to a third party.
      2. Data items provided to third parties
      3. Means or method of providing to third parties
      4. To stop providing personal information to third parties at the request of the individual
2. Notwithstanding the provisions of the preceding paragraph, the following cases shall not be deemed to be third parties.
   1. When our hospital outsources all or part of the handling of personal information to the extent necessary to achieve the purpose of use.
   2. When personal information is provided in connection with business succession due to a merger or other reason.
   3. When personal information is used jointly with a specific person, the individual is notified in advance or made easily accessible to the individual of the fact, the items of personal information to be used jointly, the scope of the joint users, the purpose of use by the users, and the name or title of the person responsible for managing the personal information.

Article 5 (Disclosure of Personal Information)

1. When an individual requests disclosure of their personal information, our hospital will disclose it to them without delay.
   However, if disclosure would result in any of the following, we may not disclose all or part of the information, and if we decide not to disclose it, we will notify you of that decision without delay.
   1. When there is a risk of harming the life, body, property, or other rights and interests of the individual or a third party
   2. When there is a risk of causing significant disruption to the proper implementation of our hospital's business
   3. If it would violate other laws and regulations.

Article 6 (Correction and Deletion of Personal Information)

1. If the personal information held by our clinic is incorrect, the user may request that our clinic correct or delete the personal information in accordance with the procedures established by our clinic.
2. If our clinic receives a request from a user as described in the preceding paragraph and determines that it is necessary to comply with the request, we will correct or delete the personal information without delay and notify the user of this.

Article 7 (Suspension of use of personal information, etc.)

If an individual requests that the use of their personal information be suspended or deleted (hereinafter referred to as "suspension of use, etc.") because their personal information is being handled beyond the scope of the intended use or because it was obtained by illegal means, the hospital will conduct the necessary investigation without delay, and based on the results, will suspend the use of their personal information and notify the individual to that effect.
However, if suspending the use of personal information would require a large amount of expense or is otherwise difficult to do, and alternative measures can be taken to protect the rights and interests of the individual, we will take these alternative measures.

Article 8 (Changes to the Privacy Policy)

The contents of this policy may be changed without notifying users.
