The Kitano Hospital, a public interest incorporated foundation run by the Tazuke Kofukai Medical Research Institute (hereinafter referred to as "our hospital"), has established the following privacy policy (hereinafter referred to as "this policy") regarding the handling of privacy information in the "No Waiting, Easy" service (hereinafter referred to as "this service") it provides.

- To display information such as name, address, contact details, credit card information, services used and their medical fees, so that users can view and correct their registered information and view their usage status.
- To use information such as name, date of birth, address, and telephone number to verify the identity of the user
- To use payment information such as the duration of treatment, number of treatments, amount billed, name, address, etc., in order to bill users for medical expenses.
- To display the information registered with our clinic on the input screen so that users can easily input data, and to transfer it to other services (including those provided by our partners) based on the user's instructions.
- The purpose of using information that can identify individuals, such as usage status, name, and address, in order to deny use to users who have violated the terms of use of this service, such as by delaying payment of medical fees or causing damage to a third party, or who attempt to use the service for fraudulent or improper purposes.
- To respond to inquiries from users, we will use information necessary for our hospital to provide services to users, such as the content of the inquiry and information related to medical fees, etc., as well as the user's service usage status and contact information.

- When it is particularly necessary for the improvement of public health or the promotion of healthy child development, and it is difficult to obtain the consent of the individual.

- If the personal information held by our clinic is incorrect, the user may request that our clinic correct or delete the personal information in accordance with the procedures established by our clinic.
- If our clinic receives a request from a user as described in the preceding paragraph and determines that it is necessary to comply with the request, we will correct or delete the personal information without delay and notify the user of this.
- Article 7 (Suspension of use of personal information, etc.)
- If an individual requests that the use of their personal information be suspended or deleted (hereinafter referred to as "suspension of use, etc.") because their personal information is being handled beyond the scope of the intended use or because it was obtained by illegal means, the hospital will conduct the necessary investigation without delay, and based on the results, will suspend the use of their personal information and notify the individual to that effect.
However, if suspending the use of personal information would require a large amount of expense or is otherwise difficult to do, and alternative measures can be taken to protect the rights and interests of the individual, we will take these alternative measures.
