Regarding the protection and use of personal information

Our foundation has established medical research facilities such as the Medical Research Institute Kitano Hospital, and is doing its utmost to strive to be a hospital that provides high-quality medical care and is trusted by patients, and that provides peace of mind by considering their situation.We believe that it is extremely important to properly protect and manage personal information.To that end, we have established the following personal information protection policy and will strive to ensure its implementation.

  1. Collection of personal information

    When the Foundation collects personal information, it will do so within the scope of the "Purpose of Use of Personal Information" section, including medical care, administrative work, management and administration, and education and research. If personal information is to be used for any other purpose, we will notify you of the purpose of use in advance and obtain your consent beforehand. The same applies when personal information is required for our website.

  2. Use and provision of personal information

    The Foundation will not use personal information beyond the scope of the purposes of use set out in "Purposes of Use of Personal Information" except in the following cases:

    • When consent is obtained from the information subject
    • When processing the information to make it impossible to identify or specify an individual.
    • When required to provide by law, etc.

    Except as required by law, the Foundation will not provide personal information to third parties without the permission of the individual who holds the information.

  3. Regarding the proper management of personal information

    The Foundation will keep personal information up to date and endeavor to prevent the leakage, loss, destruction, falsification, or unauthorized access to personal information.

  4. Regarding confirmation and correction of personal information

    If the subject of personal information requests disclosure, the Foundation will promptly confirm the details and respond in accordance with the Foundation's "Personal Information Protection Regulations." In addition, if a request is made to correct the details because they are not factual, the Foundation will investigate and respond appropriately.

  5. Inquiry desk

    Questions regarding our personal information protection policy or inquiries regarding personal information should be directed to the Medical Services Department of the Medical Research Institute Kitano Hospital, which is operated by our foundation.

  6. Compliance with laws and regulations and improvement of personal information protection systems

    The Foundation will comply with Japanese laws and regulations regarding the protection of personal information, and will review the above items as appropriate to continually improve its personal information protection system.

A third party is an organization or individual other than the information subject and the Foundation, which is the business operator, that does not fall under the Foundation's published purposes of use, or that has not obtained the consent of the information subject to use the personal information.
This policy applies to all patients who visit medical institutions operated by the Foundation, including those undergoing medical checkups and comprehensive medical examinations. It also applies to the handling of personal information of Foundation executives and employees and all individuals associated with the Foundation.

Participation in the National Clinical Database (NCD) project

Our hospital participates in the database project run by the National Clinical Database (NCD), a general incorporated association.

This project aims to improve the quality of medical care by registering, compiling, and analyzing surgery and treatment information from across Japan, thereby providing the best possible medical care to patients.

Through the activities of this corporation, we will be able to consider the optimal allocation of doctors to provide more appropriate medical care to patients, as well as obtain information that will serve as a reference for our hospital in providing the best medical care to patients. We ask for your understanding of the purpose and cooperation.

Information to be registered in NCD

We register information about surgeries and treatments performed at our hospital, as well as information (age, height, weight, etc.) used to verify the effectiveness and risks of surgeries and treatments. Patient names are not registered with NCD; instead, registration is done using an ID unrelated to the patient's name. The correspondence table linking IDs to patients is strictly managed by our hospital and is not provided to NCD.

Management of registered information and publication of results

Although the information itself does not easily identify individual patients, it is important information related to patients and will be managed with strict confidentiality. When managing the registered information, our hospital and NCD comply with laws and regulations regarding the handling and safety management of information (such as the Personal Information Protection Act, Ethical Guidelines for Medical Research Involving Human Subjects, and Guidelines for Safety Management of Medical Information Systems). When publishing data, only information approved by NCD will be published as aggregated data. The registered data will not be identified.

Rejection of registration and confirmation of registration information

If you do not want your data to be registered, you can refuse registration. Please inform our staff. Also, if you wish to view or delete your registered data, please inform our staff. Please note that refusing to register or requesting to view or modify your data will not result in any disadvantage to you in your daily medical treatment.

Cooperation in confirming registered data by visiting NCD staff

To confirm whether the information registered by our hospital with NCD is correct, NCD staff may view patients' medical charts and medical records. When our hospital cooperates with this survey, we will enter into a confidentiality agreement with NCD staff, and will prohibit the taking out of the hospital or disclosing information that can identify patients, such as patient-ID correspondence tables or names.
