Issuance of a medical certificate

If you need documents such as a medical certificate, please consult with the Document Reception Desk (④) on the 1st floor of the Main Building.
A document fee will be charged for each document created, and it will take approximately two weeks to create the documents (however, it may take longer depending on the content, etc.).

Issuance of referral letters to other hospitals

Please consult your doctor when you visit our hospital, or ask at the reception desk of each outpatient block.

Prescription refills

Prescription reissues are accepted at the patient's own expense, so please inquire at the reception desk of your outpatient block.The prescription is valid for 4 days, including the date of issue.So please be careful.
