It is estimated that there are currently around 20 million people in Japan who have diabetes or are at risk of developing it. Diabetes can now be considered a national disease, but there are a wide variety of reasons for developing diabetes (type of diabetes), physical conditions, complications, etc., and treatment varies from person to person.
Diabetes is also a disease that requires long-term living. Self-management in daily life, such as improving diet and exercise habits, taking oral medication and insulin injections, is very important. However, just as each person's physical condition is different, each person's life is also different, including their lifestyle, work, family, and personal life. I believe that there will be times in the course of a long life when recovery and treatment do not go well.
At the "Diabetes Care Support Outpatient Clinic," nurses with specialized diabetes knowledge provide consultation and guidance to patients and their families living with diabetes, so that they can live a life of recuperation while managing their diabetes in a way that is more true to themselves. While the focus is primarily on providing care support to those with type 1 or type 2 diabetes who are receiving insulin injections, the clinic also provides care support for those with gestational diabetes, as well as guidance on preventing dialysis and foot care to prevent complications.
We work in collaboration with a variety of professionals both inside and outside the hospital, including the patient's doctor, registered dietitian, pharmacist, physical therapist, administrative and social worker, and take into consideration the patient's lifestyle, striving to provide support that ideally enables them to live a better life while managing diabetes.

Kitano Hospital began offering smoking cessation outpatient services in October 2001 for patients who wish to quit smoking. The smoking cessation support outpatient service is staffed by nurses who have undergone training in smoking cessation support and are certified as smoking cessation supporters.
Many people may think, "I can't quit smoking because I have a weak will..." However, it has recently become clear that smoking is a disease of nicotine addiction, and that psychological and habitual addictions on top of that make it difficult to quit smoking. At our clinic, we work with doctors to implement smoking cessation programs based on standard smoking cessation procedures, with the aim of helping patients successfully quit smoking.
We want to support patients who have a variety of thoughts, such as "I want to quit smoking," "I want to quit smoking but I don't know how," or "I'm trying to quit smoking but I can't succeed," and help them achieve their goal of quitting smoking on their own. A stoma is an artificial opening created by surgically directing the digestive tract or urinary tract outside the body (mainly into the abdomen). It is also called an artificial anus or artificial bladder. At our stoma outpatient clinic, we provide support regarding stoma care so that people with stomas can live more comfortable daily lives. We mainly provide advice on daily life, skin care methods, introduce new products, and suggest the best stoma appliances.
We will accommodate each person for 30 minutes to 1 hour. Reservations are required, so please arrive before your scheduled time.
During the outpatient visit, we will change your appliance and check the condition of your stoma and skin, and whether there are any problems with using the appliance, so please bring your current stoma appliance and other items with you.

This refers to swelling in the arms, legs, and other parts of the body due to poor lymphatic flow. This condition is more likely to occur after lymph node removal during cancer surgery or radiation therapy. There are also primary conditions where the cause is unclear.

At our foot care nursing outpatient clinic, we provide foot care at our own expense to patients who are experiencing problems with their feet, such as being unable to cut their own nails after hip or knee surgery, having thick nails that are difficult to cut, or having calluses or corns that are painful and make it difficult to walk. Many patients who suffer from recurring calluses and corns have foot deformities such as hallux valgus as the cause, and changing their footwear can help alleviate the symptoms. We also offer advice on choosing and wearing footwear.
The staff in charge are foot care instructors and have completed training from the Japan Foot Care Society.
Anyone wishing to see a doctor must first obtain instructions and permission from a doctor. Reservations can be made with instructions from a doctor in the dermatology, plastic surgery, or orthopedic department. Feet are so important that they are sometimes called the second heart. Please take advantage of our outpatient clinic to receive proper foot care and ensure you can walk on your own two feet no matter how old you are.

We provide outpatient nursing care to patients with chronic kidney disease (CKD) and their families.
Nurses with specialized knowledge will explain how to slow the progression of chronic kidney disease and what precautions to take in daily life so that you can gain a deeper understanding.
One of the complications of diabetes is nephropathy, and we provide preventive dialysis guidance to slow its progression.
Furthermore, if kidney failure occurs, renal replacement therapy (hemodialysis, peritoneal dialysis, or kidney transplant) will be required to supplement the kidney's function. We will support patients by helping them carefully consider the characteristics of each treatment and lifestyle changes.
I try to take as much time as possible to talk individually with patients about any questions they might not have had the opportunity to ask in detail during their regular doctor's examinations, or about anything they are unsure of where to turn for advice on.
Educational hospitalization involves a one-week hospital stay for instruction and consultation for patients diagnosed with chronic kidney disease (CKD).
If you are already undergoing treatment but would like to review or consult about your lifestyle habits, please consult your outpatient doctor about educational hospitalization.

The Kitano Hospital Department of Hematology provides transplant medicine and hospitalizes patients undergoing hematopoietic stem cell transplants. Patients' treatment continues not only during their hospital stay but also after discharge. We wanted to respond to a variety of consultations, including those regarding the long-term administration of immunosuppressants, infectious diseases, worries about daily life and reintegration into society, and concerns about managing their own health.
Therefore, in April 2018, we opened the "Post-Allogeneic Transplant Outpatient Clinic (LTFU Outpatient Clinic)" with the aim of providing support as a team, focusing on the lives of patients after they are discharged from the hospital.
The nurses in charge of outpatient care after allogeneic transplants are staff who work in the hematology ward and have received the appropriate training in transplant medicine. First, we will interview patients about their progress after the transplant and monitor their physical condition, such as the condition of their skin, oral cavity, and dry eyes. Based on this information, we will provide specialized explanations about preventing post-transplant infections and complications, depending on the patient's lifestyle and the content of their consultation.
We hope to use the consultations we receive through our nursing outpatient clinic as an opportunity to review the nursing support provided during post-transplant hospitalization and to improve the quality of life after discharge.
For us nurses, meeting patients after they have been discharged from the hospital is a very joyful and precious experience.
