In order to advance medical care and improve the quality of health, medical, and welfare services, our hospital provides patients' medical information to research institutions and other organizations after anonymizing it through certified businesses recognized by the government in accordance with the Next Generation Medical Infrastructure Act.
This work has been entrusted to the General Incorporated Association Life Data Initiative (LDI), which has then subcontracted it to NTT DATA Corporation. However, due to a malfunction in the system created by NTT DATA,Medical information that should not have been provided was mistakenly shared with a certified business operator, etc. (*)We have received a report from the certified business operator stating that the report to the Cabinet Office has been completed, so we would like to inform you of this.
(*) The corporate names of certified businesses, etc. are as follows:
- Certified business operator: Life Data Initiative General Incorporated Association
- Certified processing business operator: NTT DATA Corporation
Patients who may be affected by this policy are those who have previously visited our hospital but have not visited us since October 1, 2021.
Regarding this matter, medical information is provided to research institutions and other organizations as statistical information and anonymously processed medical information that has been processed so that individuals cannot be identified or restored, and we have confirmed that no information that could lead to the identification of individual patients (such as names or addresses) has been leaked, and that there is no collaboration with any parties other than certified businesses, etc. Furthermore, we have not confirmed at this time that any secondary damage has occurred to patients.

In response to this incident, we are currently suspending the provision of data to LDI. We take this situation seriously and have formulated measures to prevent recurrence. We will request regular reports from LDI and NTT DATA on the status of handling personal information and the implementation of safety management measures (measures against unauthorized access, leaks, etc.), and will thoroughly supervise them.
