I want to book an initial consultation
A referral letter from another medical institution is required. If you do not have a referral letter, please ask your medical institution to write one for you.
When you ask your doctor to write a referral letter, you can also ask them to make an appointment at Kitano Hospital, which will make the process smoother as the two medical institutions can coordinate appointments.

Lost or expired prescription
Since it is not covered by insurance, you will need to pay for the reissue. Please call the outpatient reception of your department through the hospital's main number (06-6312-1221).
Please note that prescriptions are valid for four days, including the date of issue.
I lost my receipt
A receipt will be issued at Accounting ③ on the 1st floor of the Main Building.
