What is a regional collaborative clinical pathway?

A community collaboration pass is a treatment plan for patients who have received treatment at our hospital, in which the patient's family doctor and a doctor at our hospital work together as "two primary physicians" to provide ongoing medical care.

Regional collaboration clinical pathways used at our hospital

Regional Cancer Care Collaboration Pass

This applies to patients who have undergone surgery at our hospital and whose condition has stabilized and will be placed under observation. Once your hospitalization for surgery is confirmed, we will find you a family doctor and create an environment where you can receive medical treatment from our hospital after you are discharged.
After being discharged from the hospital, specialized medical treatments such as CT scans will be performed at our hospital, while routine medical treatments such as prescriptions will be performed by your local family doctor.
Our hospital works closely with your family doctor to exchange information, so you can receive high-quality medical care with peace of mind.

Heart Failure Regional Collaboration Pass

We use HeartNote and self-management charts to manage heart failure for patients. We are currently working to ensure that the same management methods and self-management charts are used throughout Osaka.
In the Department of Cardiology, we use the Heart Failure Regional Collaboration Pass to enable the outpatient department, local community, wards, and each department to continuously support each patient's information.
We will work together to manage your condition so that it does not worsen. We will support you so that you can continue to manage your condition.

Nephrology CKD Regional Collaboration Path

With the policy of providing patients with chronic kidney disease with the peace of mind of having two primary care physicians examine them, we have created and are using a regional collaboration clinical pathway as a tool for smooth collaboration between the patient's family doctor and Kitano Hospital. We provide patients with a "For those receiving treatment for chronic kidney disease" notebook. This booklet allows patients to record their own medical history to help them understand their own physical condition. We support patients so that they can receive medical examinations with peace of mind.
