To the patient
 
(1) When you are uncertain which department you should go to, please visit General Internal Medicine first. Please bring a referral letter from your regular physician. Without a referral letter, there will be a special care surcharge (\8,400). We also request that you bring all available medical data, as it is very important for us to know your medical history.

(2) If you keep a list of medication (gokusuri techoh made by a pharmacist), please bring it with you. In addition, be sure to let us know about any and all drugs you take, including health supplements. Without knowing the exact names of the medicines you take, we cannot diagnose drug interactions or side effects, so please provide all relevant information.

(3) For patients with long medical histories, bringing a concise, detailed memo regarding their condition will be helpful to us. Please let us know about your symptoms and concerns as well as medical care you have received elsewhere. Taking the particulars of your previous medical examinations, diagnoses, and treatment into account will help us provide the best care for your individual situation.

(4) We provide English-language consultations; however, please let us know if you would like an interpreter for another language present. We can make arrangements with advance notice.

(5) Kitano Hospital promotes regional medical cooperation. We would like you to use Kitano Hospital General Internal Medicine within a regional medical network to help resolve new medical afflictions that are not yet diagnosed. If a course of treatment is determined at the General Internal Medicine clinic, we would like for you to return to your regular physician for further treatments. If you do not have one, we can make a referral, so please ask our medical coordinator.
